New Coronavirus Infectious Vaccination Certificate (For Japan / Overseas)
We will issue a "New Coronavirus Infectious Vaccination Certificate" that officially proves the fact that you have been vaccinated against the new coronavirus infections.
There are two types of vaccination certificates, "for overseas" and "for Japan", each of which can be issued electronically or on paper form.
My Number Card is required to apply for the vaccination certificate (electronic version). You can read your My Number Card, apply for it, and obtain it with a dedicated smartphone app that can read your My Number Card.
It takes about one month from application to receipt of the My Number Card. If you have not obtained a My Number Card and are considering obtaining an vaccination certificate (electronic version), please apply as soon as possible so that it will be in time for the date of use.

※ For use in Japan, the vaccination
certificate and vaccination record can still be used as documents to prove the
fact of vaccination.

Target people
Those who have received the new coronavirus vaccine based on the Immunization Act using the vaccination coupon issued by Nagahama City
The following people are not eligible for issuance.
[Those not covered by the Immunization Act]
• Temporary returnees of Japanese nationals living overseas
• U.S. military personnel in Japan who received U.S. military vaccinations
• Domestic study participants
• Those who received the vaccination overseas

What you need to obtain an electronic version
(1) Smartphone
・Devices that can read My Number cards (NFC Type B compatible devices)
・iOS 13.7 or higher
・ Android OS 8.0 or higher
(2) My number card (You also need to enter the 4-digit PIN number for assistance on the face of the card set when receiving the card)
(3) Passport (only for overseas applications)

How to apply/acquire
Download the "New Corona Vaccination Certificate App" from your smartphone, and follow the steps on the app screen to apply and obtain it.
In order to verify your identity, you will need to enter the 4-digit PIN (PIN to assist you in entering information on the face of the card) that you set when you received your My Number Card and scan your My Number Card. (If you apply for a certificate for overseas use, you will also need to scan your passport.)
The acquired vaccination certificate can be displayed at any time by launching the app.

・The municipality selected on the app is the municipality with a resident card at the time of vaccination. If you moved between the 1st and 2nd vaccination, please select each municipality and apply.

・If the certificate of vaccination cannot be issued electronically, you can apply for a certificate of vaccination (issued in paper form) by mail or at the counter.

● Those who have a maiden name, another name, or an alias
●Persons requiring consideration such as DV victims
● Those who have not registered their vaccination record or who need to correct their vaccination record
