A vaccination coupon is required to receive vaccinations. If any of the following apply, the vaccination coupon will be reissued, so please complete the necessary procedures.

• If the vaccination coupon is lost, destroyed, damaged, etc.

• Those eligible for vaccination who have moved from another municipality and have not completed the vaccination

• When the vaccination coupon is used only for a preliminary examination (not used for vaccination)

• If you have received one to three vaccinations of a combination of overseas and domestic vaccinations (only if you have been vaccinated with Pfizer, Moderna, or AstraZeneca vaccines).

The reissued vaccination coupon will be sent by ordinary mail after the application. The delivery address will be the address listed on your resident card.

If you would like it mailed to a different address, please let us know.

If you find the lost vaccination coupon after reissuing it, please discard it as it cannot be used.
