2. NOTIFICATION OF TRANSFER (TENSHUTSU TODOKE)／転出（英語）

(1) Eligible person                                                                    

People whose address is transferred from Nagahama city to other municipalities (Nagahama city → outside the city)

(2) Person who has a notification obligation

Within 14 days from the date of transfer

(4)Things to bring                                                                    

1. Those who can verify the identity of the person who comes to the counter

※My number card, Driver's license, Insurance card, Residence card, Passport etc.

2. National Health Insurance Card (if you have one）

3. Nursing-care insurance card (if you have one)
4. Late-stage elderly medical insurance card (if you have one)

5. Welfare medical expenses receipt ticket (if you have one)
6. When an agent makes a notification, a proxy authorization notice (power of attorney) for moving the address

The person to be transferred can do the transfer procedure by mail.

Please prepare the following necessary items and send them to Nagahama City Hall Citizens’ Affairs Division (Shiminka).

After the transfer procedure, we will mail the transfer certificate to the address of the transfer destination.

When you receive the transfer certificate, please proceed the transfer with the mailed transfer certificate at the city, town, village of the transfer place.

B  Special exception moving-out In case of mailing                                    

Exceptional case: If the person is transferred out by mail, please send a transfer notice (mailing) of the use of My number card etc. to Nagahama City Hall.

After completing the transfer procedure in Nagahama City, you can transfer to the city, town and village of the transfer place with My number card or Resident basic register card. When the transfer procedure in Nagahama city is completed, we will contact the notification person from the Nagahama City Hall, and then we will ask the municipalities of the transfer place to proceed transfer procedures.
