3. NOTIFICATION OF TRANSFER(TENKYO TODOKE) ／転居（英語）

People whose address is transferred within Nagahama city (city → city).

　・Principal

　・Head of household before address change

Within 14 days before and after the expected transfer date.

1. A copy of verifying yourself

　・My number card, Driver's license, Insurance card, Residence card, Passport etc.

2. My number card or Resident basic registration card with face photo (if you have one).

3. In case of foreign nationality, the residence card or special permanent resident certificate of all people who need to change is required.

4.National Health Insurance Card (if you have one)

5. Long-Term Care Insurance Card (if you have one)

6. Late-stage elderly medical insurance card (if you have one)

7. Welfare medical expenses receipt ticket (if you have one)

8. When a representative makes a notification, a notice of proxy authorization (power of attorney) for address change.
